Are you ready to change lives? The Senior Friendship Centers has an opening for a dynamic The Assessment Coordinator provides screening and assessment services for home delivered meal applicants and supports congregate meal dining site operations and additional nutrition programs.
This position meets with seniors at their residential location to assess what programs and assistance are available to the senior.
Must be fluent in English and Spanish and be able to communicate with patience and compassion when interviewing seniors.
Candidates must be comfortable driving to multiple locations with their personal vehicle.
The Assessment Coordinator also acts as an on-call staff member for the meal dining sites.

org Essential Functions: Home Delivered Meals Respond to referrals for home delivered meals (HDM) and congregate meals.
Administer the DOEA 701A/701C assessment form electronically and complete all case file documentation.
Initiate, coordinate, and monitor meal service plans with clients acting as liaison with agencies.
Contact home delivered meals waitlisted clients to determine continued interest.
Complete data entry, client reports and verify all information.
Assist with meal orders, weekly reconciliation, and billing functions.
Develop and promote relationships with service providers and businesses to assure understanding of the organizations mission.
Develop and distribute nutrition outreach and educational materials to promote the program.
Congregate Meals Manage the daily operations of a congregate meal dining site as needed.
Receive food as delivered to the site, check temperatures, and arrange for substitute service when necessary.
Responsible for the set-up, clean up, and sanitation of the kitchen and dining area.
Additional site maintenance or building cleaning may be assigned depending on location.
Ensure supplies are properly stored, and kitchen and storage areas are secured at end of shift.
Record daily temperatures of the refrigerator and freezer.
Ensure program functions remain compliant with Area Agency on Aging, Department of Elder Affairs, and county regulations and policies.
Supervise, train, and direct the work of volunteers.
Work closely with vendors to assure menu compliance, quality control, and timely delivery.
Oversee and reconcile the accurate ordering and serving of meals; prepare and take reservations and cancellations.
Operational Assist with requests and monitoring of the program by the Area Agency on Aging, the Department of Elder Affairs, and the Department of Human Services as needed.
Assist with additional administrative records and tasks for the Nutrition Program.
Report any incident of abuse or neglect as mandated by state guidelines.
Must comply with HIPAA regulations, OSHA laws, and safety protocols.
Organizational: Attend staff meetings, training, and seminars as required.
Participate in fundraising and outreach activities to promote the organizations mission.
Must comply with all Senior Friendship Centers personnel policies.
Must protect the safety and health of staff, volunteers, participants, and others by observing and following the organizations safety and incident policies.

Education, Experience, Licenses, and Certifications: Requires a degree in Social Work, Human Services, or a related field.
Management experience is strongly preferred.
Experience presenting social, recreational, and educational programs to adults is desirable.
Intermediate computer skills are required; Must have experience using Microsoft Office Suite (Word, Excel, Outlook) and other basic computer software and hardware, and the ability to quickly learn the organizations systems.
Bilingual required (English/Spanish).
Physical Demands: Prolonged periods sitting at a desk and working on a computer and standing.
Must be able to lift 30 pounds.
Must be able to navigate various departments of a physical location.
Travel: Daily travel is required to complete assessments and operate congregate meal site locations.
Travel within the several counties in which Senior Friendship Centers Inc.
operates may be required as frequently as needed to support operations, fundraising, and outreach.
Must have reliable transportation.
A valid drivers license and proof of insurance are required for all work-related travel.
Additional eligibility requirements: Satisfactory clearance of a fingerprint-based, level II background screening and pre-employment substance abuse testing is required.
